So firstly download the file here:

In your app, under Application click the hamburger, select Field Viewers, then go to What's Next / Import XML and select the file you downloaded.

Now that's installed, go to your form and add a field of type Text (Paragraphs), and under Display Settings select the field viewer in the Custom Field Viewer setting.

Then publish and you should see it in your form.
